  1. List of company shareholders in mining companies up to the Beneficial Ownership (Last Beneficiary)

  2. Copy of:
    1. Work Order from the Commitment Maker Official if implementing a government/local government project; or
    2. Work Order from the Authorized Official if implementing a state-owned enterprise/local government-owned enterprise project; or
    3. Location Permit from the relevant authority in accordance with the applicant's investment plan, in accordance with the provisions of the prevailing laws and regulations, if conducting the applicant's project

  3. Approval letter for the activity plan from the permit-issuing agency in accordance with the field of business based on KBLI

  4. Transportation and sale/utilization plan and the duration of activities that produce excavated minerals/coal.
    Note: The applicant must attach a Google Earth aerial photo showing a clear overhead view of the project location.
