   • Type of Food Processing Place (select the appropriate one: Jasa Boga A/B/C, restaurant, specific TPP, Drinking Water Depot)
   • Name of the Food Processing Place
   • Address of the Food Processing Place
   • Number of food handlers or (for Drinking Water Depots) the number of depot operators
   • Number of food handlers with a certificate of food safety training or (for Drinking Water Depots) a certificate of hygiene and sanitation training for depot operators

2. Technical Requirements include:
   • Certificate of food safety training for the manager/owner/responsible person of TPP
   • Certificate of food safety training for food handlers or hygiene and sanitation training for Drinking Water Depot food handlers/operator

3. Evidence of Standardized Quality Standards issued by a KAN-accredited laboratory or a laboratory designated by the regional government no later than 1 month before the application

4. HEALTH ENVIRONMENT INSPECTION FORM (choose one):
   • JASA BOGA/KATERING (Catering Services)
   • RESTORAN (Restaurant)
