Requirements:

1. Specific Requirements:
   • Specific to signal amplification equipment (repeater/booster) for mobile communication systems, attach a cooperation agreement with Indonesian mobile network operators;
   • Specific to satellite phones/modems, attach a cooperation agreement with Indonesian telecommunication service providers offering satellite phone/modem services;
   • Specific to mobile phone, handheld computer, and tablet devices, attach a declaration of security assurance for International Mobile Equipment Identity (IMEI) and a list of IMEIs approved by the Global System for Mobile Communications Association (GSMA) or equivalent;
   • Specific to certificate applications for telecommunication devices submitted by Indonesian businesses appointed as official representatives or distributors by foreign brand owners, attach a letter of appointment as an official representative or distributor from the brand owner;
   • Specific to certificate applications for telecommunication devices submitted by legal Indonesian brand owners, attach documents demonstrating compliance with relevant regulations related to brand ownership from the minister responsible for legal and human rights affairs;
   • Specific to telecommunication devices obligated to comply with industry-specific regulations, attach documents demonstrating compliance with relevant regulations related to the telecommunication devices from the minister responsible for industrial affairs.

2. General Requirements:
   • Test Report issued by domestic testing institutions designated or foreign testing institutions recognized by the minister responsible for communication and informatics affairs;
   • Technical specifications documents for telecommunication devices;
   • Declaration of Conformity to technical standards of telecommunication devices signed by the applicant;
   • Color photo of the telecommunication device displaying brand data and model type;
   • Statement that the data and documents provided are true and valid;
   • Payment of telecommunication device certificate fees.

Obligations:

1. Attach a label containing information about the business entity's identity, telecommunication device certificate number, and a warning against making modifications that would lead to non-compliance with the specified technical standards; Duration: -
2. Report evidence of label production as mentioned in point a to the minister responsible for communication and informatics affairs; Duration: -
3. Submit changes to the telecommunication device certificate in case of changes in administrative data, such as:
   • Change of the certificate holder's name;
   • Change of the certificate holder's address.
