Requirements
- Scan the original letter of application from the Rector/Chairman regarding the establishment of Off-Campus Study Program (PSDKU) to the Minister of Education and Culture
- Scan the original Notarial Deed of Establishment of the Organizer Body along with any changes, if any have been made
- Scan the original Minister of Law and Human Rights Decree on the ratification of the Organizer Body as a Legal Entity
- Scan the original Decree of the Minister of National Education/Minister of Education and Culture/Minister of Research, Technology and Higher Education (Mendikbudristek) on the establishment/operation permit of Private Higher Education (PTS) and the permit for opening study programs in the main campus that will be conducted off the main campus
- Scan the original certificate of excellence or A ranking in study programs at the main campus
- Scan the original letter of consent from the organizing body regarding the opening of Off-Campus Study Program (PSDKU)
- Scan the original letter of consideration from the University Senate regarding the opening of Off-Campus Study Program (PSDKU)
- Scan the original Cooperation Agreement with State Universities (PTN) or Private Higher Education (PTS) accredited in the provincial area where Off-Campus Study Program (PSDKU) will be opened
- Scan the original cooperation agreement with State Universities (PTN) or Private Higher Education (PTS) accredited in the provincial area where Off-Campus Study Program (PSDKU) will be opened
- Scan the original ownership certificate, building use rights, or land use rights in the location of Off-Campus Study Program (PSDKU) implementation / Lease agreement with the land rights holder in the location of Off-Campus Study Program (PSDKU) with an area that matches the needs of the study program to be opened
- Scan the original written recommendation from the Higher Education Service Institute (LLDIKTI) in the area where the main campus is located and/or the Higher Education Service Institute (LLDIKTI) in the area of the Off-Campus Study Program (PSDKU) to be opened
- Instrument for Meeting the Minimum Accreditation Requirements for Off-Campus Study Program (PSDKU)
- Attachment of the Instrument for Meeting the Minimum Accreditation Requirements for Off-Campus Study Program (PSDKU)
- Scan the original strategic plan of the university consisting of the cover page, table of contents, and pages that contain the plan for the opening of Off-Campus Study Program (PSDKU)
- Scan the written recommendation from the Regent/Mayor regarding the potential and interest of prospective students in the Off-Campus Study Program (PSDKU) to be opened
- Degree and Transcript of Prospective Permanent Lecturers
- Letter of Agreement to Become a Permanent Lecturer
- ID Card or Passport for All Prospective Lecturers
- Curriculum Vitae for All Prospective Lecturers
