Requirements

1. Technical Requirements:

• Detailed design drawings of the type or structure to be constructed, technical specifications, as well as the schedule and implementation methods such as site plans, map placements, location photos, 1:1000 site maps, and more;
• Risk analysis;
• Implementation methods and traffic control;
• Environmental studies; and/or
• Types of infrastructure and technology to be used.

2. Administrative Requirements:

• Name, occupation, and address of the applicant;
• Purpose and objectives of utilizing road sections;
• Plan for utilizing road sections, including location, length of work in kilometers, cable diameter, and more;
• Duration of utilization of road sections required;
• Environmental permit and approval of environmental impact analysis or environmental permit and recommendations for environmental management and monitoring, or a statement of environmental management and monitoring capabilities from the competent authority;
• Statement in the provided form (signed on duty stamp);
• Lease agreement or permit statement (related to State-Owned Assets leasing);
• Copy of applicant's ID card;
• Copy of the Articles of Association and any amendments;
• Taxpayer Identification Number (NPWP); and/or
• Valid Taxpayer Status Certificate.

Obligations

• 1. Minutes of Coordination with Relevant Authorities;
• 2. Compliance with the terms and conditions for the utilization and use of toll road sections;
• 3. Minutes with relevant agencies such as PAM, Telkom, etc., to avoid disruption to existing utilities;
• 4. Letter from the local Government related to the levy for advertising or media (IPR) arrangements; and
• 5. Requirements stated in the Technical Permit/Recommendation issued.
